abstract = {This research explored associations between helmet use and head injuries in snowsports by investigating reported snowsport injuries in Western Canada from 2008-2009 to 2012-2013. The key finding was that increased helmet use (from 69% to 80%) was not associated with a reduction in reported head injuries. Over the study period, the average rate of reported head injuries was 0.2/1000 skier visits, with a statistically significant variation ( P \< 0.001). The line of best fit showed an non-significant upward trend ( P = 0.13). Lacerations were the only subcategory of head injuries that decreased significantly with helmet use. A higher proportion of people who reported a head injury were wearing a helmet than for injuries other than to the head. Skiers were more likely to report a head injury when wearing a helmet than snowboarders ( P \< 0.001 cf. P = 0.22). There were significant differences in characteristics of helmet and non-helmet wearers. Helmet wearers were more likely to be: young adults ( P \< 0.001); beginner/novices ( P = 0.004); and snowboarders ( P \< 0.001), but helmet wearing was not associated with gender ( P = 0.191). Further research is needed to explore the possible reasons for the failure of helmets to reduce head injuries, for example, increased reporting of head injuries and increased risk-taking combined with over-rating of the helmets' protection.},

abstract = {Attention deficit hyperactivity disorder (ADHD) has been identified as a risk factor for concussion or delayed recovery. However, these recommendations are often based on anecdotal or limited evidence. This critically appraised topic aimed to address the clinical question of whether a premorbid diagnosis of ADHD increases the risk for a concussion and prolonged symptoms. A search strategy using PICO terminology related to the clinical question was conducted in several electronic databases. Seven studies were identified and five met the inclusion criteria. The clinical bottom line suggests there is moderate evidence to support the relationship between patients who are diagnosed as having ADHD and the incident rate of concussion, as well as a prolonged recovery time. The findings of this critically appraised topic suggest strength of recommendation of grade B to support that ADHD is a risk factor for concussion and a predictor of prolonged recovery. Secondary aims were examining if player's maturity in age, history of concussion and years played and parents who have undergone first aid and concussion training would increase knowledge. Methods: 1,441 parents and 284 youth ARF players completed a 23-item concussion knowledge survey consisting of three areas: concussion symptoms, management, and return-to-play criteria. Results: There was significant difference in knowledge of concussion management and return-to play criteria between players and parents (p= less than 0.01). Players age, years played and history of concussion did not increase knowledge, however, parents with concussion training had significantly higher scores than those without (p= less than 0.01). Some return-to-play criteria and signs and symptom not thought to be associated with concussion such as disturbed sleeping and difficulty studying were difficult for both groups to associate as related to concussion. Conclusions: Future concussion education opportunities for player and parents need to focus on return-to-play criteria and uncommon signs and symptoms such as sleep disturbances and problems studying. Applications in sport: The findings reflect certain areas that need to be incorporated into educational opportunities for players and parents, especially those who have limited access to qualified health care professionals such as ATC's. Although there is minimal evidence supporting concussion educational opportunities, awareness and knowledge of concussion is the greatest positive influence for symptom reporting among young athletes. However, the ideal delivery mode and educational content for interventions appropriate to each group has yet to be identified so steps such as defining the target group, measuring their level of awareness and knowledge, and monitoring of effectiveness. Overall, the goal should be to provide at least the basic information regarding concussion but as the results of this study demonstrate ensure lesser known aspects such as return-to-play criteria and signs and symptoms such as sleep disturbances and difficulty studying or concentrating are incorporated. abstract = {Background: Sports-related concussions (SRCs) have gained increased societal interest in the past decade. The National Collegiate Athletic Association (NCAA) has implemented legislation and rule changes to decrease the incidence and risk of head injury impacts. The “targeting” rule forbids initiating contact with the crown of a helmet and targeting defenseless players in the head and neck area; however, there are concerns that this rule change has unintentionally led to an increased incidence of lower extremity injuries. Purpose/Hypothesis: The purpose of this study was to evaluate the change in lower extremity injury rates in NCAA football during the 2009-2010 to 2014-2015 seasons. We hypothesized that the lower extremity injury rate has increased across the time period. Study Design: Descriptive epidemiology study. Methods: Sixty-eight NCAA football programs provided 153 team-seasons of data to the NCAA Injury Surveillance Program. Lower extremity injuries (ie, hip/groin, upper leg/thigh, knee, lower leg/Achilles, foot/toes) and SRCs sustained during NCAA football games were examined. We calculated injury rates per 1000 athlete-exposures (AEs) for lower extremity injuries and SRCs. Rate ratios (RRs) compared injury rates between the 2009-2010 to 2011-2012 and 2012-2013 to 2014-2015 seasons. Results: Overall, 2400 lower extremity injuries were reported during the 2009-2010 to 2014-2015 seasons; most were to the knee (33.6%) and ankle (28.5%) and caused by player contact (59.2%). The lower extremity injury rate increased in 2012-2013 to 2014-2015 compared with 2009-2010 to 2011-2012 (23.55 vs 20.45/1000 AEs, respectively; RR, 1.15; 95% CI, 1.06-1.25). This finding was retained when restricted to injuries due to player contact (RR, 1.19; 95% CI, 1.07-1.32) but not for injuries due to noncontact/overuse (RR, 0.96; 95% CI, 0.80-1.14). When examining player contact injury rates by anatomic site, only ankle injuries had an increase (RR, 1.36; 95% CI, 1.13-1.64). The SRC rate also increased in 2012-2013 to 2014-2015 compared with 2009-2010 to 2011-2012 (3.52 vs 2.63/1000 AEs, respectively; RR, 1.34; 95% CI, 1.08-1.66). Conclusion: The lower extremity injury rate has increased in NCAA football athletes. Similarly, SRC rates have increased, although this may be caused by concurrent policies related to better education, identification, and management. Targeting rule changes may be contributing to increased rates of player contact\textendashrelated ankle injuries. Alongside continued surveillance research to examine longitudinal time trends, more in-depth individual-level examinations of how targeting rule changes influence coaching and player behaviors are warranted. abstract = {Objective: The objective of this paper is to review existing literature surrounding the utility of the King-Devick test which is a commonly used sideline assessment tool for sport-related concussions. Methods: A review of the literature was performed using MEDLINE, CINHAL, and SportDiscus databases. The search was performed from the beginning of the record through November 16th, 2015. Results: This search strategy yielded 27 articles from aforementioned databases. Further searching in The Cochrane Library with King-Devick AND Concuss* search terms yielded one additional article, summing a total of 28 articles. After removal of duplicates and implementation of the inclusion/exclusion criteria, 8 articles for extensively reviewed. Conclusion: This narrative review suggests that the King-Devick test is an efficient sideline assessment tool for sport-related concussions. However, we recommend that the King-Devick should be used as a sideline screening tool, not a concussion diagnosis tool at this time. A proper baseline time including multiple tests may be recommended to negate the learning affect and to have a reliable baseline in which to measure from for future reference. A three second difference appears appropriate to identify the possibility of concussion and to remove an athlete from play. At this time, the athlete should be monitored and further evaluated as symptoms are sometimes delayed. We suggest that further research may be useful to better determine the efficacy of the K-D test in detecting concussions across a broader range of athletes and sports. We also suggest further research may investigate the K-D test a potential return-to-play tool for clinicians and medical personnel.},

abstract = {Objective: To examine publicly-available information on all identified cases of suicide in active or former American professional football players between 1920 and the spring of 2015. Design: Retrospective cohort study. Setting: Professional American Football in the US. Participants: A cohort of 26 702 athletes who had died, retired or were currently playing in the NFL from nfl.com since 1920 was identified. Main outcome measures: Internet queries identifying 26 professional football players who completed suicide. Obituaries and news reports were reviewed. The primary outcome measures included mortality, demographic characteristics and life circumstances in professional American football players completing suicide. Results: From 1920\textendash2015, the median age of the 26 men who completed suicide was 39.5 years (range = 23\textendash85). The median number of years after retirement was 6.5 (range = 0\textendash63). Most of the deaths since 1920 have occurred in the past 15 years (58.7%) and a large percentage have occurred since 2009 (42.3%). Most of the men suffered from multiple life stressors prior to their deaths, such as retirement from sport, loss of steady income, divorce, failed business ventures, estrangement from family members and medical, psychiatric and/or substance abuse problems. Conclusions: A disproportionate number of completed suicides in current and former professional football players have occurred since 2009 (42.3%). It is well established in the literature that the causes of depression and suicidality are diverse, often multifactorial and treatable. Providing at-risk retired athletes with mental health treatment will likely reduce their suffering and improve their quality-of-life. [ABSTRACT FROM AUTHOR]},

abstract = {Objective: To examine the influence of age, sex, attention-deficit hyperactivity disorder (ADHD) status, previous history of concussion, and days since injury on postconcussion postural control assessment in adolescents who have suffered a concussion. Design: Prospective cohort study. Setting: Hospital-based outpatient clinic. Participants: Seventy-one participants (42 males; 29 females) with mean age 14.14 ± 2.44. Independent Variables: Age, sex, previous concussion history, ADHD status, total and severity of postconcussion symptoms, and days since injury. Main Outcome Measures: Total Balance Error Scoring System score, path length, center-of-pressure (COP) area, sample entropy, and Romberg quotient. Results: Pearson product-moment correlation coefficients were calculated to test for potential associations between the continuous participant characteristics and the postural control variables. Spearman correlation was used to test the association between symptom severity and the postural control variables. Standard multiple regression was used to model the extent to which participant characteristics accounted for the variance in the postural sway variables. Age was significantly associated with all of the postural sway variables except COP area for the eyes open condition and sample entropy in the anterior-posterior direction for the eyes closed condition. Sex, ADHD status, and previous concussion history did not significantly predict postural control scores. Conclusions: Age significantly influences scores on common postconcussion postural control assessments. Clinical Relevance: This study demonstrates that age is a critical factor that needs to be accounted for to improve the clinical appropriateness and utility of current postconcussion postural control assessments. abstract = {Neurocognitive test batteries are commonly used tools for concussion assessments in the medical professions. Administered at baseline and post-injury these tests provide information on a patient's neurocognitive ability during the recovery and return-to-activity phases. In athletics, student-athletes usually take the baseline exam as a group in a computer lab prior to the season beginning. If a medical professional believes an individual has sustained a possible concussion, they will retest them and compare their post-injury and baseline results. A deficit in one of more areas of the neurocognitive test may be indicative of a possible concussion. The purpose of this study was to examine whether or not there was a difference in neurocognitive test scores from Concussion Vital Signs when tested under two different conditions: "lights out" and with distractions. Our study was a randomized control trial performed at a Midwestern NCAA, "mid-major" Division I Institution. The subjects were 15 college-aged students with an overall mean age of 19 years (...1.2). There were seven females (mean age was 19 ± 0.77) and eight males (mean age was 20 ± 1.2). A paired t-test was used to determine if a difference in the neurocognitive test section scores between the conditions existed. On three of the ten test sections, there was a statistically significant difference between the baseline and distraction condition. On four of the ten test sections between baseline and the lights out condition for males, but not for females. Testing should be done in a quiet room, with distractions minimized, as distraction hinders focus and performance. The results of this study indicate males may have a more difficult time concentrating while taking neurocognitive tests than females. abstract = {This study examined factors that predicted the existence and comprehensiveness of w ritten concussion management policies in secondary schools. A purposeful sample of athletic trainers employed in secondary schools was requested to complete an online survey (1,124 of 3,943 [28.5% response rate]). Most athletic trainers (968 of 1,112 [87.1%]) reported having a written concussion management policy, which included background information (883 of 968 [91.2%]), immediate treatment (902 of 968 [93.2%]), and return to play criteria (933 of 968 [96.4%]). Strongest predictive variables of an existing concussion policy included athletic trainers who were: primarily paid via school/ district (odds ratio [OR] = 2.1,95% confidence interval [Cl] = 1.4 to 3.3), hired full-time (OR = 1.7,95% Cl = 1.1 to 2.7), and evaluation of more concussions annually (OR = 1.6,95% Cl = 1.3 to 2.0). abstract = {Purpose: The decision regarding return to activity (RTA) after mild traumatic brain injuries/concussion is one of the most difficult and controversial areas in concussion management, particularly for youth. This study investigated how youth with postconcussion syndrome (PCS) are affected by exertion and whether standardized exertion testing using the McMaster All-Out Progressive Continuous Cycling Test can contribute to clinical decision making for safe RTA. Methods: Fifty-four youth (8.5-18.3 yr) with a previously confirmed concussion participated in the study. Each participant performed exertion testing on a cycle ergometer and completed a Postconcussion Symptom scale at the following time points: before exertion (baseline), 5 and 30 min, and 24 h after exertion. A modified Postconcussion Symptom scale was administered at 2-min intervals during exertion. Results: Participants had a mean T SD symptom duration of 6.3 T 6.9 months after the most recent concussive injury, with a median of 4.1 months (range, 0.7-35 months). Sixty-three percent of participants had symptoms during exertion testing. Symptom profile (number and severity) significantly affected perception of exertion at 50% peak mechanical power. During acute assessment of symptoms (30-min after exertion), headache (P = 0.39), nausea (P = 0.63), and dizziness (P = 0.35) did not change. However, both the number and severity of symptoms significantly improved over 24 h, with 56.8% of youth showing improvements. The time from the most recent injury had a significant effect on the symptom score at baseline, 30 min after exertion, and 24 h after exertion. Conclusions: Exertion testing has an important role in the evaluation of symptoms and readiness to RTA, particularly in youth who are slow to recover. abstract = {Context: Computerized neuropsychological testing is commonly used in the assessment and management of sport-related concussion. Even though computerized testing is widespread, psychometric evidence for test-retest reliability is somewhat limited. Additional evidence for test-retest reliability is needed to optimize clinical decision making after concussion. Objective: To document test-retest reliability for a commercially available computerized neuropsychological test battery (ImPACT) using 2 different clinically relevant time intervals. Design: Cross-sectional study. Setting: Two research laboratories. Patients or Other Participants: Group 1 (n=46) consisted of 25 men and 21 women (age=22.4 ± 1.89 years). Group 2 (n = 45) consisted of 17 men and 28 women (age = 20.9 ± 1.72 years). Intervention(s): Both groups completed ImPACT forms 1, 2, and 3, which were delivered sequentially either at 1-week intervals (group 1) or at baseline, day 45, and day 50 (group 2). Group 2 also completed the Green Word Memory Test (WMT) as a measure of effort. Main Outcome Measures: Intraclass correlation coefficients (ICCs) were calculated for the composite scores of ImPACT between time points. Repeated-measures analysis of variance was used to evaluate changes in ImPACT and WMT results over time. Results: The ICC values for group 1 ranged from 0.26 to 0.88 for the 4 ImPACT composite scores. The ICC values for group 2 ranged from 0.37 to 0.76. In group 1, ImPACT classified 37.0% and 46.0% of healthy participants as impaired at time points 2 and 3, respectively. In group 2, ImPACT classified 22.2% and 28.9% of healthy participants as impaired at time points 2 and 3, respectively. Conclusions: We found variable test-retest reliability for ImPACT metrics. Visual motor speed and reaction time demonstrated greater reliability than verbal and visual memory. Our current data support a multifaceted approach to concussion assessment using clinical examinations, symptom reports, cognitive testing, and balance assessment.},

abstract = {Abstract: Objectives: To investigate the relation of postconcussive complaints, anxiety, and depression with vocational outcome in patients with traumatic brain injury (TBI) of various severities and to assess sex differences. Design: A prospective cross-sectional cohort study. Setting: Level I trauma center. Participants: Adults (N=242) with TBI of various severity. Interventions: Not applicable. Main Outcome Measures: Extended Glasgow Outcome Scale, return to work (RTW), Head Injury Symptom Checklist, and Hospital Anxiety and Depression Scale. Results: In 67% of the patients, complaints were present; 22% were anxious, and 18% were depressed. The frequency of complaints increased significantly with injury severity, in contrast to anxiety and depression. Frequencies of patients with anxiety and depression (9% and 5%) were lower with complete RTW than with incomplete RTW (42% and 37%; P\<.001). Patients with minor TBI with complaints were more anxious (50% vs 27%; P\<.05) and depressed (46% vs 23%; P\<.05) compared with patients with other severity categories and patients with incomplete RTW (67% vs 36% and 60% vs 30%, respectively). A higher percentage of women with minor TBI were depressed (45% vs 13%; P=.01) and had incomplete RTW (50% vs 18%; P\<.05) compared with men. Multiple regression analysis showed that injury severity, complaints, anxiety, and depression were all predictive of RTW (explained variance 45%). In all severity categories, anxiety and depression were predictive of RTW, complaints, and sex only for minor TBI. abstract = {Context: Analysis of health care service models requires the collection and evaluation of basic practice characterization data. Practice-based research networks (PBRNs) provide a framework for gathering data useful in characterizing clinical practice. Objective: To describe preliminary secondary school setting practice data from the Athletic Training Practice-Based Research Network (AT-PBRN). Design: Descriptive study. Setting: Secondary school athletic training facilities within the AT-PBRN. Patients or Other Participants: Clinicians (n = 22) and their patients (n = 2523) from the AT-PBRN. Main Outcome Measure(s): A Web-based survey was used to obtain data on clinical practice site and clinician characteristics. Patient and practice characteristics were obtained via deidentified electronic medical record data collected between September 1, 2009, and April 1, 2011. Descriptive data regarding the clinician and CPS practice characteristics are reported as percentages and frequencies. Descriptive analysis of patient encounters and practice characteristic data was performed, with the percentages and frequencies of the type of injuries recorded at initial evaluation, type of treatment received at initial evaluation, daily treatment, and daily sign-in procedures. Results: The AT-PBRN had secondary school sites in 7 states, and most athletic trainers at those sites (78.2%) had less than 5 years of experience. The secondary school sites within the AT-PBRN documented 2523 patients treated across 3140 encounters. Patients most frequently sought care for a current injury (61.3%), followed by preventive services (24.0%), and new injuries (14.7%). The most common diagnoses were ankle sprain/strain (17.9%), hip sprain/strain (12.5%), concussion (12.0%), and knee pain (2.5%). The most frequent procedures were athletic trainer evaluation (53.9%), hot- or cold-pack application (26.0%), strapping (10.3%), and therapeutic exercise (5.7%). The median number of treatments per injury was 3 (interquartile range = 2, 4; range = 2-19). Conclusions: These preliminary data describe services provided by clinicians within the AT-PBRN and demonstrate the usefulness of the PBRN model for obtaining such data.},

abstract = {BACKGROUND: Most reports on in-line skating injuries describe severe injuries admitted to injury clinics. Little is known about the overall rate of injury and the contribution of the different disciplines. METHODS: In a retrospective inquiry data on injuries were obtained of 105 in-line skaters of whose 69% were active in fitness skating, 59% in jumping/grinding and 51% in the halfpipe. Beside the nature, location and degree of the injury, information was given on where the injury happened and whether protectors were worn at the time of injury. RESULTS: Of 197 injuries 145 healed without medical treatment, 28 needed medical advice once, 22 several times and 2 needed hospitalisation. Injury location were 38% in the upper, 31% in the lower extremities, 21% in the hip/pelvis region and 10% on the head. The most frequent injuries pertained to concussions and skin lesions, 35% each, followed by ligament injuries with 10% and fractures with 5%. Simple injuries were 83% in fitness skating, 70% in jumping/grinding and 60% in the halfpipe. Several visits were necessary for 4% of fitness skaters, 10% of jumpers/grinders and 23% in halfpipe injured persons. All four protectors were worn at the time of injury by 5% in fitness skating, 18% in jumping/grinding and 55% in the halfpipe. No protectors were worn in 26% of the fitness injuries, in 9% jumping/grinding and in 6% in the halfpipe. CONCLUSIONS: The results reveal an apparent discipline specific degree of danger and that fitness in-line skating is less dangerous than the resulting benefits for preventive medicine.},

abstract = {This research explored associations between helmet use and head injuries in snowsports by investigating reported snowsport injuries in Western Canada from 2008-2009 to 2012-2013. The key finding was that increased helmet use (from 69% to 80%) was not associated with a reduction in reported head injuries. Over the study period, the average rate of reported head injuries was 0.2/1000 skier visits, with a statistically significant variation ( P \< 0.001). The line of best fit showed an non-significant upward trend ( P = 0.13). Lacerations were the only subcategory of head injuries that decreased significantly with helmet use. A higher proportion of people who reported a head injury were wearing a helmet than for injuries other than to the head. Skiers were more likely to report a head injury when wearing a helmet than snowboarders ( P \< 0.001 cf. P = 0.22). There were significant differences in characteristics of helmet and non-helmet wearers. Helmet wearers were more likely to be: young adults ( P \< 0.001); beginner/novices ( P = 0.004); and snowboarders ( P \< 0.001), but helmet wearing was not associated with gender ( P = 0.191). Further research is needed to explore the possible reasons for the failure of helmets to reduce head injuries, for example, increased reporting of head injuries and increased risk-taking combined with over-rating of the helmets' protection.},

abstract = {Attention deficit hyperactivity disorder (ADHD) has been identified as a risk factor for concussion or delayed recovery. However, these recommendations are often based on anecdotal or limited evidence. This critically appraised topic aimed to address the clinical question of whether a premorbid diagnosis of ADHD increases the risk for a concussion and prolonged symptoms. A search strategy using PICO terminology related to the clinical question was conducted in several electronic databases. Seven studies were identified and five met the inclusion criteria. The clinical bottom line suggests there is moderate evidence to support the relationship between patients who are diagnosed as having ADHD and the incident rate of concussion, as well as a prolonged recovery time. The findings of this critically appraised topic suggest strength of recommendation of grade B to support that ADHD is a risk factor for concussion and a predictor of prolonged recovery. abstract = {Purpose: The purpose of this study was to measure concussion knowledge and awareness of youth Australian Rules Football players and parents. Secondary aims were examining if player's maturity in age, history of concussion and years played and parents who have undergone first aid and concussion training would increase knowledge. Methods: 1,441 parents and 284 youth ARF players completed a 23-item concussion knowledge survey consisting of three areas: concussion symptoms, management, and return-to-play criteria. Results: There was significant difference in knowledge of concussion management and return-to play criteria between players and parents (p= less than 0.01). Players age, years played and history of concussion did not increase knowledge, however, parents with concussion training had significantly higher scores than those without (p= less than 0.01). Some return-to-play criteria and signs and symptom not thought to be associated with concussion such as disturbed sleeping and difficulty studying were difficult for both groups to associate as related to concussion. Conclusions: Future concussion education opportunities for player and parents need to focus on return-to-play criteria and uncommon signs and symptoms such as sleep disturbances and problems studying. Applications in sport: The findings reflect certain areas that need to be incorporated into educational opportunities for players and parents, especially those who have limited access to qualified health care professionals such as ATC's. Although there is minimal evidence supporting concussion educational opportunities, awareness and knowledge of concussion is the greatest positive influence for symptom reporting among young athletes. However, the ideal delivery mode and educational content for interventions appropriate to each group has yet to be identified so steps such as defining the target group, measuring their level of awareness and knowledge, and monitoring of effectiveness. Overall, the goal should be to provide at least the basic information regarding concussion but as the results of this study demonstrate ensure lesser known aspects such as return-to-play criteria and signs and symptoms such as sleep disturbances and difficulty studying or concentrating are incorporated. abstract = {Background: Sports-related concussions (SRCs) have gained increased societal interest in the past decade. The National Collegiate Athletic Association (NCAA) has implemented legislation and rule changes to decrease the incidence and risk of head injury impacts. The “targeting” rule forbids initiating contact with the crown of a helmet and targeting defenseless players in the head and neck area; however, there are concerns that this rule change has unintentionally led to an increased incidence of lower extremity injuries. Purpose/Hypothesis: The purpose of this study was to evaluate the change in lower extremity injury rates in NCAA football during the 2009-2010 to 2014-2015 seasons. We hypothesized that the lower extremity injury rate has increased across the time period. Study Design: Descriptive epidemiology study. Methods: Sixty-eight NCAA football programs provided 153 team-seasons of data to the NCAA Injury Surveillance Program. Lower extremity injuries (ie, hip/groin, upper leg/thigh, knee, lower leg/Achilles, foot/toes) and SRCs sustained during NCAA football games were examined. We calculated injury rates per 1000 athlete-exposures (AEs) for lower extremity injuries and SRCs. Rate ratios (RRs) compared injury rates between the 2009-2010 to 2011-2012 and 2012-2013 to 2014-2015 seasons. Results: Overall, 2400 lower extremity injuries were reported during the 2009-2010 to 2014-2015 seasons; most were to the knee (33.6%) and ankle (28.5%) and caused by player contact (59.2%). The lower extremity injury rate increased in 2012-2013 to 2014-2015 compared with 2009-2010 to 2011-2012 (23.55 vs 20.45/1000 AEs, respectively; RR, 1.15; 95% CI, 1.06-1.25). This finding was retained when restricted to injuries due to player contact (RR, 1.19; 95% CI, 1.07-1.32) but not for injuries due to noncontact/overuse (RR, 0.96; 95% CI, 0.80-1.14). When examining player contact injury rates by anatomic site, only ankle injuries had an increase (RR, 1.36; 95% CI, 1.13-1.64). The SRC rate also increased in 2012-2013 to 2014-2015 compared with 2009-2010 to 2011-2012 (3.52 vs 2.63/1000 AEs, respectively; RR, 1.34; 95% CI, 1.08-1.66). Conclusion: The lower extremity injury rate has increased in NCAA football athletes. Similarly, SRC rates have increased, although this may be caused by concurrent policies related to better education, identification, and management. Targeting rule changes may be contributing to increased rates of player contact\textendashrelated ankle injuries. Alongside continued surveillance research to examine longitudinal time trends, more in-depth individual-level examinations of how targeting rule changes influence coaching and player behaviors are warranted. abstract = {Objective: The objective of this paper is to review existing literature surrounding the utility of the King-Devick test which is a commonly used sideline assessment tool for sport-related concussions. Methods: A review of the literature was performed using MEDLINE, CINHAL, and SportDiscus databases. The search was performed from the beginning of the record through November 16th, 2015. Results: This search strategy yielded 27 articles from aforementioned databases. Further searching in The Cochrane Library with King-Devick AND Concuss* search terms yielded one additional article, summing a total of 28 articles. After removal of duplicates and implementation of the inclusion/exclusion criteria, 8 articles for extensively reviewed. Conclusion: This narrative review suggests that the King-Devick test is an efficient sideline assessment tool for sport-related concussions. However, we recommend that the King-Devick should be used as a sideline screening tool, not a concussion diagnosis tool at this time. A proper baseline time including multiple tests may be recommended to negate the learning affect and to have a reliable baseline in which to measure from for future reference. A three second difference appears appropriate to identify the possibility of concussion and to remove an athlete from play. At this time, the athlete should be monitored and further evaluated as symptoms are sometimes delayed. We suggest that further research may be useful to better determine the efficacy of the K-D test in detecting concussions across a broader range of athletes and sports. We also suggest further research may investigate the K-D test a potential return-to-play tool for clinicians and medical personnel.},

abstract = {Objective: To examine publicly-available information on all identified cases of suicide in active or former American professional football players between 1920 and the spring of 2015. Design: Retrospective cohort study. Setting: Professional American Football in the US. Participants: A cohort of 26 702 athletes who had died, retired or were currently playing in the NFL from nfl.com since 1920 was identified. Main outcome measures: Internet queries identifying 26 professional football players who completed suicide. Obituaries and news reports were reviewed. The primary outcome measures included mortality, demographic characteristics and life circumstances in professional American football players completing suicide. Results: From 1920\textendash2015, the median age of the 26 men who completed suicide was 39.5 years (range = 23\textendash85). The median number of years after retirement was 6.5 (range = 0\textendash63). Most of the deaths since 1920 have occurred in the past 15 years (58.7%) and a large percentage have occurred since 2009 (42.3%). Most of the men suffered from multiple life stressors prior to their deaths, such as retirement from sport, loss of steady income, divorce, failed business ventures, estrangement from family members and medical, psychiatric and/or substance abuse problems. Conclusions: A disproportionate number of completed suicides in current and former professional football players have occurred since 2009 (42.3%). It is well established in the literature that the causes of depression and suicidality are diverse, often multifactorial and treatable. abstract = {Objective: To examine the influence of age, sex, attention-deficit hyperactivity disorder (ADHD) status, previous history of concussion, and days since injury on postconcussion postural control assessment in adolescents who have suffered a concussion. Design: Prospective cohort study. Setting: Hospital-based outpatient clinic. Participants: Seventy-one participants (42 males; 29 females) with mean age 14.14 ± 2.44. Independent Variables: Age, sex, previous concussion history, ADHD status, total and severity of postconcussion symptoms, and days since injury. Main Outcome Measures: Total Balance Error Scoring System score, path length, center-of-pressure (COP) area, sample entropy, and Romberg quotient. Results: Pearson product-moment correlation coefficients were calculated to test for potential associations between the continuous participant characteristics and the postural control variables. Spearman correlation was used to test the association between symptom severity and the postural control variables. Standard multiple regression was used to model the extent to which participant characteristics accounted for the variance in the postural sway variables. Age was significantly associated with all of the postural sway variables except COP area for the eyes open condition and sample entropy in the anterior-posterior direction for the eyes closed condition. Sex, ADHD status, and previous concussion history did not significantly predict postural control scores. Conclusions: Age significantly influences scores on common postconcussion postural control assessments. Clinical Relevance: This study demonstrates that age is a critical factor that needs to be accounted for to improve the clinical appropriateness and utility of current postconcussion postural control assessments. abstract = {Neurocognitive test batteries are commonly used tools for concussion assessments in the medical professions. Administered at baseline and post-injury these tests provide information on a patient's neurocognitive ability during the recovery and return-to-activity phases. In athletics, student-athletes usually take the baseline exam as a group in a computer lab prior to the season beginning. If a medical professional believes an individual has sustained a possible concussion, they will retest them and compare their post-injury and baseline results. A deficit in one of more areas of the neurocognitive test may be indicative of a possible concussion. The purpose of this study was to examine whether or not there was a difference in neurocognitive test scores from Concussion Vital Signs when tested under two different conditions: "lights out" and with distractions. Our study was a randomized control trial performed at a Midwestern NCAA, "mid-major" Division I Institution. The subjects were 15 college-aged students with an overall mean age of 19 years (...1.2). There were seven females (mean age was 19 ± 0.77) and eight males (mean age was 20 ± 1.2). A paired t-test was used to determine if a difference in the neurocognitive test section scores between the conditions existed. On three of the ten test sections, there was a statistically significant difference between the baseline and distraction condition. On four of the ten test sections between baseline and the lights out condition for males, but not for females. Testing should be done in a quiet room, with distractions minimized, as distraction hinders focus and performance. The results of this study indicate males may have a more difficult time concentrating while taking neurocognitive tests than females. A purposeful sample of athletic trainers employed in secondary schools was requested to complete an online survey (1,124 of 3,943 [28.5% response rate]). Most athletic trainers (968 of 1,112 [87.1%]) reported having a written concussion management policy, which included background information (883 of 968 [91.2%]), immediate treatment (902 of 968 [93.2%]), and return to play criteria (933 of 968 [96.4%]). Strongest predictive variables of an existing concussion policy included athletic trainers who were: primarily paid via school/ district (odds ratio [OR] = 2.1,95% confidence interval [Cl] = 1.4 to 3.3), hired full-time (OR = 1.7,95% Cl = 1.1 to 2.7), and evaluation of more concussions annually (OR = 1.6,95% Cl = 1.3 to 2.0). abstract = {Purpose: The decision regarding return to activity (RTA) after mild traumatic brain injuries/concussion is one of the most difficult and controversial areas in concussion management, particularly for youth. This study investigated how youth with postconcussion syndrome (PCS) are affected by exertion and whether standardized exertion testing using the McMaster All-Out Progressive Continuous Cycling Test can contribute to clinical decision making for safe RTA. Methods: Fifty-four youth (8.5-18.3 yr) with a previously confirmed concussion participated in the study. Each participant performed exertion testing on a cycle ergometer and completed a Postconcussion Symptom scale at the following time points: before exertion (baseline), 5 and 30 min, and 24 h after exertion. A modified Postconcussion Symptom scale was administered at 2-min intervals during exertion. Results: Participants had a mean T SD symptom duration of 6.3 T 6.9 months after the most recent concussive injury, with a median of 4.1 months (range, 0.7-35 months). Sixty-three percent of participants had symptoms during exertion testing. Symptom profile (number and severity) significantly affected perception of exertion at 50% peak mechanical power. During acute assessment of symptoms (30-min after exertion), headache (P = 0.39), nausea (P = 0.63), and dizziness (P = 0.35) did not change. However, both the number and severity of symptoms significantly improved over 24 h, with 56.8% of youth showing improvements. The time from the most recent injury had a significant effect on the symptom score at baseline, 30 min after exertion, and 24 h after exertion. Conclusions: Exertion testing has an important role in the evaluation of symptoms and readiness to RTA, particularly in youth who are slow to recover. abstract = {Context: Computerized neuropsychological testing is commonly used in the assessment and management of sport-related concussion. Even though computerized testing is widespread, psychometric evidence for test-retest reliability is somewhat limited. Additional evidence for test-retest reliability is needed to optimize clinical decision making after concussion. Objective: To document test-retest reliability for a commercially available computerized neuropsychological test battery (ImPACT) using 2 different clinically relevant time intervals. Design: Cross-sectional study. Setting: Two research laboratories. Patients or Other Participants: Group 1 (n=46) consisted of 25 men and 21 women (age=22.4 ± 1.89 years). Group 2 (n = 45) consisted of 17 men and 28 women (age = 20.9 ± 1.72 years). Intervention(s): Both groups completed ImPACT forms 1, 2, and 3, which were delivered sequentially either at 1-week intervals (group 1) or at baseline, day 45, and day 50 (group 2). Group 2 also completed the Green Word Memory Test (WMT) as a measure of effort. Main Outcome Measures: Intraclass correlation coefficients (ICCs) were calculated for the composite scores of ImPACT between time points. Repeated-measures analysis of variance was used to evaluate changes in ImPACT and WMT results over time. Results: The ICC values for group 1 ranged from 0.26 to 0.88 for the 4 ImPACT composite scores. The ICC values for group 2 ranged from 0.37 to 0.76. In group 1, ImPACT classified 37.0% and 46.0% of healthy participants as impaired at time points 2 and 3, respectively. In group 2, ImPACT classified 22.2% and 28.9% of healthy participants as impaired at time points 2 and 3, respectively. Conclusions: We found variable test-retest reliability for ImPACT metrics. Visual motor speed and reaction time demonstrated greater reliability than verbal and visual memory. Our current data support a multifaceted approach to concussion assessment using clinical examinations, symptom reports, cognitive testing, and balance assessment.},

abstract = {Abstract: Objectives: To investigate the relation of postconcussive complaints, anxiety, and depression with vocational outcome in patients with traumatic brain injury (TBI) of various severities and to assess sex differences. Design: A prospective cross-sectional cohort study. Setting: Level I trauma center. Participants: Adults (N=242) with TBI of various severity. Interventions: Not applicable. Main Outcome Measures: Extended Glasgow Outcome Scale, return to work (RTW), Head Injury Symptom Checklist, and Hospital Anxiety and Depression Scale. Results: In 67% of the patients, complaints were present; 22% were anxious, and 18% were depressed. The frequency of complaints increased significantly with injury severity, in contrast to anxiety and depression. Frequencies of patients with anxiety and depression (9% and 5%) were lower with complete RTW than with incomplete RTW (42% and 37%; P\<.001). Patients with minor TBI with complaints were more anxious (50% vs 27%; P\<.05) and depressed (46% vs 23%; P\<.05) compared with patients with other severity categories and patients with incomplete RTW (67% vs 36% and 60% vs 30%, respectively). A higher percentage of women with minor TBI were depressed (45% vs 13%; P=.01) and had incomplete RTW (50% vs 18%; P\<.05) compared with men. Multiple regression analysis showed that injury severity, complaints, anxiety, and depression were all predictive of RTW (explained variance 45%). In all severity categories, anxiety and depression were predictive of RTW, complaints, and sex only for minor TBI. Conclusions: Anxiety and depression are related to vocational outcome after TBI, with a different profile in the minor TBI category, partly due to sex differences. Copyright \&y\& Elsevier},

abstract = {Context: Analysis of health care service models requires the collection and evaluation of basic practice characterization data. Practice-based research networks (PBRNs) provide a framework for gathering data useful in characterizing clinical practice. Objective: To describe preliminary secondary school setting practice data from the Athletic Training Practice-Based Research Network (AT-PBRN). Design: Descriptive study. Setting: Secondary school athletic training facilities within the AT-PBRN. Patients or Other Participants: Clinicians (n = 22) and their patients (n = 2523) from the AT-PBRN. Main Outcome Measure(s): A Web-based survey was used to obtain data on clinical practice site and clinician characteristics. Patient and practice characteristics were obtained via deidentified electronic medical record data collected between September 1, 2009, and April 1, 2011. Descriptive data regarding the clinician and CPS practice characteristics are reported as percentages and frequencies. Descriptive analysis of patient encounters and practice characteristic data was performed, with the percentages and frequencies of the type of injuries recorded at initial evaluation, type of treatment received at initial evaluation, daily treatment, and daily sign-in procedures. Results: The AT-PBRN had secondary school sites in 7 states, and most athletic trainers at those sites (78.2%) had less than 5 years of experience. The secondary school sites within the AT-PBRN documented 2523 patients treated across 3140 encounters. Patients most frequently sought care for a current injury (61.3%), followed by preventive services (24.0%), and new injuries (14.7%). The most common diagnoses were ankle sprain/strain (17.9%), hip sprain/strain (12.5%), concussion (12.0%), and knee pain (2.5%). The most frequent procedures were athletic trainer evaluation (53.9%), hot- or cold-pack application (26.0%), strapping (10.3%), and therapeutic exercise (5.7%). The median number of treatments per injury was 3 (interquartile range = 2, 4; range = 2-19). Conclusions: These preliminary data describe services provided by clinicians within the AT-PBRN and demonstrate the usefulness of the PBRN model for obtaining such data.},

abstract = {This research explored associations between helmet use and head injuries in snowsports by investigating reported snowsport injuries in Western Canada from 2008-2009 to 2012-2013. The key finding was that increased helmet use (from 69% to 80%) was not associated with a reduction in reported head injuries. Over the study period, the average rate of reported head injuries was 0.2/1000 skier visits, with a statistically significant variation ( P \< 0.001). The line of best fit showed an non-significant upward trend ( P = 0.13). Lacerations were the only subcategory of head injuries that decreased significantly with helmet use. A higher proportion of people who reported a head injury were wearing a helmet than for injuries other than to the head. Skiers were more likely to report a head injury when wearing a helmet than snowboarders ( P \< 0.001 cf. P = 0.22). There were significant differences in characteristics of helmet and non-helmet wearers. Helmet wearers were more likely to be: young adults ( P \< 0.001); beginner/novices ( P = 0.004); and snowboarders ( P \< 0.001), but helmet wearing was not associated with gender ( P = 0.191). Further research is needed to explore the possible reasons for the failure of helmets to reduce head injuries, for example, increased reporting of head injuries and increased risk-taking combined with over-rating of the helmets' protection.},

abstract = {Attention deficit hyperactivity disorder (ADHD) has been identified as a risk factor for concussion or delayed recovery. However, these recommendations are often based on anecdotal or limited evidence. This critically appraised topic aimed to address the clinical question of whether a premorbid diagnosis of ADHD increases the risk for a concussion and prolonged symptoms. A search strategy using PICO terminology related to the clinical question was conducted in several electronic databases. Seven studies were identified and five met the inclusion criteria. The clinical bottom line suggests there is moderate evidence to support the relationship between patients who are diagnosed as having ADHD and the incident rate of concussion, as well as a prolonged recovery time. The findings of this critically appraised topic suggest strength of recommendation of grade B to support that ADHD is a risk factor for concussion and a predictor of prolonged recovery. Secondary aims were examining if player's maturity in age, history of concussion and years played and parents who have undergone first aid and concussion training would increase knowledge. Methods: 1,441 parents and 284 youth ARF players completed a 23-item concussion knowledge survey consisting of three areas: concussion symptoms, management, and return-to-play criteria. Results: There was significant difference in knowledge of concussion management and return-to play criteria between players and parents (p= less than 0.01). Players age, years played and history of concussion did not increase knowledge, however, parents with concussion training had significantly higher scores than those without (p= less than 0.01). Some return-to-play criteria and signs and symptom not thought to be associated with concussion such as disturbed sleeping and difficulty studying were difficult for both groups to associate as related to concussion. Conclusions: Future concussion education opportunities for player and parents need to focus on return-to-play criteria and uncommon signs and symptoms such as sleep disturbances and problems studying. Applications in sport: The findings reflect certain areas that need to be incorporated into educational opportunities for players and parents, especially those who have limited access to qualified health care professionals such as ATC's. Although there is minimal evidence supporting concussion educational opportunities, awareness and knowledge of concussion is the greatest positive influence for symptom reporting among young athletes. However, the ideal delivery mode and educational content for interventions appropriate to each group has yet to be identified so steps such as defining the target group, measuring their level of awareness and knowledge, and monitoring of effectiveness. Overall, the goal should be to provide at least the basic information regarding concussion but as the results of this study demonstrate ensure lesser known aspects such as return-to-play criteria and signs and symptoms such as sleep disturbances and difficulty studying or concentrating are incorporated. abstract = {Background: Sports-related concussions (SRCs) have gained increased societal interest in the past decade. The National Collegiate Athletic Association (NCAA) has implemented legislation and rule changes to decrease the incidence and risk of head injury impacts. The “targeting” rule forbids initiating contact with the crown of a helmet and targeting defenseless players in the head and neck area; however, there are concerns that this rule change has unintentionally led to an increased incidence of lower extremity injuries. Purpose/Hypothesis: The purpose of this study was to evaluate the change in lower extremity injury rates in NCAA football during the 2009-2010 to 2014-2015 seasons. We hypothesized that the lower extremity injury rate has increased across the time period. Study Design: Descriptive epidemiology study. Methods: Sixty-eight NCAA football programs provided 153 team-seasons of data to the NCAA Injury Surveillance Program. Lower extremity injuries (ie, hip/groin, upper leg/thigh, knee, lower leg/Achilles, foot/toes) and SRCs sustained during NCAA football games were examined. We calculated injury rates per 1000 athlete-exposures (AEs) for lower extremity injuries and SRCs. Rate ratios (RRs) compared injury rates between the 2009-2010 to 2011-2012 and 2012-2013 to 2014-2015 seasons. Results: Overall, 2400 lower extremity injuries were reported during the 2009-2010 to 2014-2015 seasons; most were to the knee (33.6%) and ankle (28.5%) and caused by player contact (59.2%). The lower extremity injury rate increased in 2012-2013 to 2014-2015 compared with 2009-2010 to 2011-2012 (23.55 vs 20.45/1000 AEs, respectively; RR, 1.15; 95% CI, 1.06-1.25). This finding was retained when restricted to injuries due to player contact (RR, 1.19; 95% CI, 1.07-1.32) but not for injuries due to noncontact/overuse (RR, 0.96; 95% CI, 0.80-1.14). When examining player contact injury rates by anatomic site, only ankle injuries had an increase (RR, 1.36; 95% CI, 1.13-1.64). The SRC rate also increased in 2012-2013 to 2014-2015 compared with 2009-2010 to 2011-2012 (3.52 vs 2.63/1000 AEs, respectively; RR, 1.34; 95% CI, 1.08-1.66). Conclusion: The lower extremity injury rate has increased in NCAA football athletes. Similarly, SRC rates have increased, although this may be caused by concurrent policies related to better education, identification, and management. Targeting rule changes may be contributing to increased rates of player contact\textendashrelated ankle injuries. Alongside continued surveillance research to examine longitudinal time trends, more in-depth individual-level examinations of how targeting rule changes influence coaching and player behaviors are warranted. abstract = {Objective: The objective of this paper is to review existing literature surrounding the utility of the King-Devick test which is a commonly used sideline assessment tool for sport-related concussions. Methods: A review of the literature was performed using MEDLINE, CINHAL, and SportDiscus databases. The search was performed from the beginning of the record through November 16th, 2015. Results: This search strategy yielded 27 articles from aforementioned databases. Further searching in The Cochrane Library with King-Devick AND Concuss* search terms yielded one additional article, summing a total of 28 articles. After removal of duplicates and implementation of the inclusion/exclusion criteria, 8 articles for extensively reviewed. Conclusion: This narrative review suggests that the King-Devick test is an efficient sideline assessment tool for sport-related concussions. However, we recommend that the King-Devick should be used as a sideline screening tool, not a concussion diagnosis tool at this time. A proper baseline time including multiple tests may be recommended to negate the learning affect and to have a reliable baseline in which to measure from for future reference. A three second difference appears appropriate to identify the possibility of concussion and to remove an athlete from play. At this time, the athlete should be monitored and further evaluated as symptoms are sometimes delayed. We suggest that further research may be useful to better determine the efficacy of the K-D test in detecting concussions across a broader range of athletes and sports. We also suggest further research may investigate the K-D test a potential return-to-play tool for clinicians and medical personnel.},

abstract = {Objective: To examine publicly-available information on all identified cases of suicide in active or former American professional football players between 1920 and the spring of 2015. Design: Retrospective cohort study. Setting: Professional American Football in the US. Participants: A cohort of 26 702 athletes who had died, retired or were currently playing in the NFL from nfl.com since 1920 was identified. Main outcome measures: Internet queries identifying 26 professional football players who completed suicide. Obituaries and news reports were reviewed. The primary outcome measures included mortality, demographic characteristics and life circumstances in professional American football players completing suicide. Results: From 1920\textendash2015, the median age of the 26 men who completed suicide was 39.5 years (range = 23\textendash85). The median number of years after retirement was 6.5 (range = 0\textendash63). Most of the deaths since 1920 have occurred in the past 15 years (58.7%) and a large percentage have occurred since 2009 (42.3%). Most of the men suffered from multiple life stressors prior to their deaths, such as retirement from sport, loss of steady income, divorce, failed business ventures, estrangement from family members and medical, psychiatric and/or substance abuse problems. Conclusions: A disproportionate number of completed suicides in current and former professional football players have occurred since 2009 (42.3%). It is well established in the literature that the causes of depression and suicidality are diverse, often multifactorial and treatable. Providing at-risk retired athletes with mental health treatment will likely reduce their suffering and improve their quality-of-life. [ABSTRACT FROM AUTHOR]},

abstract = {Objective: To examine the influence of age, sex, attention-deficit hyperactivity disorder (ADHD) status, previous history of concussion, and days since injury on postconcussion postural control assessment in adolescents who have suffered a concussion. Design: Prospective cohort study. Setting: Hospital-based outpatient clinic. Participants: Seventy-one participants (42 males; 29 females) with mean age 14.14 ± 2.44. Independent Variables: Age, sex, previous concussion history, ADHD status, total and severity of postconcussion symptoms, and days since injury. Main Outcome Measures: Total Balance Error Scoring System score, path length, center-of-pressure (COP) area, sample entropy, and Romberg quotient. Results: Pearson product-moment correlation coefficients were calculated to test for potential associations between the continuous participant characteristics and the postural control variables. Spearman correlation was used to test the association between symptom severity and the postural control variables. Standard multiple regression was used to model the extent to which participant characteristics accounted for the variance in the postural sway variables. Age was significantly associated with all of the postural sway variables except COP area for the eyes open condition and sample entropy in the anterior-posterior direction for the eyes closed condition. Sex, ADHD status, and previous concussion history did not significantly predict postural control scores. Conclusions: Age significantly influences scores on common postconcussion postural control assessments. Clinical Relevance: This study demonstrates that age is a critical factor that needs to be accounted for to improve the clinical appropriateness and utility of current postconcussion postural control assessments. abstract = {Neurocognitive test batteries are commonly used tools for concussion assessments in the medical professions. Administered at baseline and post-injury these tests provide information on a patient's neurocognitive ability during the recovery and return-to-activity phases. In athletics, student-athletes usually take the baseline exam as a group in a computer lab prior to the season beginning. If a medical professional believes an individual has sustained a possible concussion, they will retest them and compare their post-injury and baseline results. A deficit in one of more areas of the neurocognitive test may be indicative of a possible concussion. The purpose of this study was to examine whether or not there was a difference in neurocognitive test scores from Concussion Vital Signs when tested under two different conditions: "lights out" and with distractions. Our study was a randomized control trial performed at a Midwestern NCAA, "mid-major" Division I Institution. The subjects were 15 college-aged students with an overall mean age of 19 years (...1.2). There were seven females (mean age was 19 ± 0.77) and eight males (mean age was 20 ± 1.2). A paired t-test was used to determine if a difference in the neurocognitive test section scores between the conditions existed. On three of the ten test sections, there was a statistically significant difference between the baseline and distraction condition. On four of the ten test sections between baseline and the lights out condition for males, but not for females. Testing should be done in a quiet room, with distractions minimized, as distraction hinders focus and performance. The results of this study indicate males may have a more difficult time concentrating while taking neurocognitive tests than females. A purposeful sample of athletic trainers employed in secondary schools was requested to complete an online survey (1,124 of 3,943 [28.5% response rate]). Most athletic trainers (968 of 1,112 [87.1%]) reported having a written concussion management policy, which included background information (883 of 968 [91.2%]), immediate treatment (902 of 968 [93.2%]), and return to play criteria (933 of 968 [96.4%]). Strongest predictive variables of an existing concussion policy included athletic trainers who were: primarily paid via school/ district (odds ratio [OR] = 2.1,95% confidence interval [Cl] = 1.4 to 3.3), hired full-time (OR = 1.7,95% Cl = 1.1 to 2.7), and evaluation of more concussions annually (OR = 1.6,95% Cl = 1.3 to 2.0). abstract = {Purpose: The decision regarding return to activity (RTA) after mild traumatic brain injuries/concussion is one of the most difficult and controversial areas in concussion management, particularly for youth. This study investigated how youth with postconcussion syndrome (PCS) are affected by exertion and whether standardized exertion testing using the McMaster All-Out Progressive Continuous Cycling Test can contribute to clinical decision making for safe RTA. Methods: Fifty-four youth (8.5-18.3 yr) with a previously confirmed concussion participated in the study. Each participant performed exertion testing on a cycle ergometer and completed a Postconcussion Symptom scale at the following time points: before exertion (baseline), 5 and 30 min, and 24 h after exertion. A modified Postconcussion Symptom scale was administered at 2-min intervals during exertion. Results: Participants had a mean T SD symptom duration of 6.3 T 6.9 months after the most recent concussive injury, with a median of 4.1 months (range, 0.7-35 months). Sixty-three percent of participants had symptoms during exertion testing. Symptom profile (number and severity) significantly affected perception of exertion at 50% peak mechanical power. During acute assessment of symptoms (30-min after exertion), headache (P = 0.39), nausea (P = 0.63), and dizziness (P = 0.35) did not change. However, both the number and severity of symptoms significantly improved over 24 h, with 56.8% of youth showing improvements. The time from the most recent injury had a significant effect on the symptom score at baseline, 30 min after exertion, and 24 h after exertion. Conclusions: Exertion testing has an important role in the evaluation of symptoms and readiness to RTA, particularly in youth who are slow to recover. abstract = {Context: Computerized neuropsychological testing is commonly used in the assessment and management of sport-related concussion. Even though computerized testing is widespread, psychometric evidence for test-retest reliability is somewhat limited. Additional evidence for test-retest reliability is needed to optimize clinical decision making after concussion. Objective: To document test-retest reliability for a commercially available computerized neuropsychological test battery (ImPACT) using 2 different clinically relevant time intervals. Design: Cross-sectional study. Setting: Two research laboratories. Patients or Other Participants: Group 1 (n=46) consisted of 25 men and 21 women (age=22.4 ± 1.89 years). Group 2 (n = 45) consisted of 17 men and 28 women (age = 20.9 ± 1.72 years). Intervention(s): Both groups completed ImPACT forms 1, 2, and 3, which were delivered sequentially either at 1-week intervals (group 1) or at baseline, day 45, and day 50 (group 2). Group 2 also completed the Green Word Memory Test (WMT) as a measure of effort. Main Outcome Measures: Intraclass correlation coefficients (ICCs) were calculated for the composite scores of ImPACT between time points. Repeated-measures analysis of variance was used to evaluate changes in ImPACT and WMT results over time. Results: The ICC values for group 1 ranged from 0.26 to 0.88 for the 4 ImPACT composite scores. The ICC values for group 2 ranged from 0.37 to 0.76. In group 1, ImPACT classified 37.0% and 46.0% of healthy participants as impaired at time points 2 and 3, respectively. In group 2, ImPACT classified 22.2% and 28.9% of healthy participants as impaired at time points 2 and 3, respectively. Conclusions: We found variable test-retest reliability for ImPACT metrics. Visual motor speed and reaction time demonstrated greater reliability than verbal and visual memory. Our current data support a multifaceted approach to concussion assessment using clinical examinations, symptom reports, cognitive testing, and balance assessment.},

abstract = {Abstract: Objectives: To investigate the relation of postconcussive complaints, anxiety, and depression with vocational outcome in patients with traumatic brain injury (TBI) of various severities and to assess sex differences. Design: A prospective cross-sectional cohort study. Setting: Level I trauma center. Participants: Adults (N=242) with TBI of various severity. Interventions: Not applicable. Main Outcome Measures: Extended Glasgow Outcome Scale, return to work (RTW), Head Injury Symptom Checklist, and Hospital Anxiety and Depression Scale. Results: In 67% of the patients, complaints were present; 22% were anxious, and 18% were depressed. The frequency of complaints increased significantly with injury severity, in contrast to anxiety and depression. Frequencies of patients with anxiety and depression (9% and 5%) were lower with complete RTW than with incomplete RTW (42% and 37%; P\<.001). Patients with minor TBI with complaints were more anxious (50% vs 27%; P\<.05) and depressed (46% vs 23%; P\<.05) compared with patients with other severity categories and patients with incomplete RTW (67% vs 36% and 60% vs 30%, respectively). A higher percentage of women with minor TBI were depressed (45% vs 13%; P=.01) and had incomplete RTW (50% vs 18%; P\<.05) compared with men. Multiple regression analysis showed that injury severity, complaints, anxiety, and depression were all predictive of RTW (explained variance 45%). In all severity categories, anxiety and depression were predictive of RTW, complaints, and sex only for minor TBI. abstract = {The Immediate Post-Concussion Assessment and Cognitive Testing (ImPACT) is reported to possess variable reliability potentially due to a variety of factors, including mood. This study examined the influence of mood on ImPACT performance. A repeated measures design was used to assess 104 healthy, college-aged students. Participants completed the Profile of Mood States-Brief (POMS-B) and the ImPACT at days 1, 45, and 50. Pearson correlation coefficients were calculated for the POMS-B and ImPACT composite scores. abstract = {Context: Analysis of health care service models requires the collection and evaluation of basic practice characterization data. Practice-based research networks (PBRNs) provide a framework for gathering data useful in characterizing clinical practice. Objective: To describe preliminary secondary school setting practice data from the Athletic Training Practice-Based Research Network (AT-PBRN). Design: Descriptive study. Setting: Secondary school athletic training facilities within the AT-PBRN. Patients or Other Participants: Clinicians (n = 22) and their patients (n = 2523) from the AT-PBRN. Main Outcome Measure(s): A Web-based survey was used to obtain data on clinical practice site and clinician characteristics. Patient and practice characteristics were obtained via deidentified electronic medical record data collected between September 1, 2009, and April 1, 2011. Descriptive data regarding the clinician and CPS practice characteristics are reported as percentages and frequencies. Descriptive analysis of patient encounters and practice characteristic data was performed, with the percentages and frequencies of the type of injuries recorded at initial evaluation, type of treatment received at initial evaluation, daily treatment, and daily sign-in procedures. Results: The AT-PBRN had secondary school sites in 7 states, and most athletic trainers at those sites (78.2%) had less than 5 years of experience. The secondary school sites within the AT-PBRN documented 2523 patients treated across 3140 encounters. Patients most frequently sought care for a current injury (61.3%), followed by preventive services (24.0%), and new injuries (14.7%). The most common diagnoses were ankle sprain/strain (17.9%), hip sprain/strain (12.5%), concussion (12.0%), and knee pain (2.5%). The most frequent procedures were athletic trainer evaluation (53.9%), hot- or cold-pack application (26.0%), strapping (10.3%), and therapeutic exercise (5.7%). The median number of treatments per injury was 3 (interquartile range = 2, 4; range = 2-19). Conclusions: These preliminary data describe services provided by clinicians within the AT-PBRN and demonstrate the usefulness of the PBRN model for obtaining such data.},

abstract = {BACKGROUND: Most reports on in-line skating injuries describe severe injuries admitted to injury clinics. Little is known about the overall rate of injury and the contribution of the different disciplines. METHODS: In a retrospective inquiry data on injuries were obtained of 105 in-line skaters of whose 69% were active in fitness skating, 59% in jumping/grinding and 51% in the halfpipe. Beside the nature, location and degree of the injury, information was given on where the injury happened and whether protectors were worn at the time of injury. RESULTS: Of 197 injuries 145 healed without medical treatment, 28 needed medical advice once, 22 several times and 2 needed hospitalisation. Injury location were 38% in the upper, 31% in the lower extremities, 21% in the hip/pelvis region and 10% on the head. The most frequent injuries pertained to concussions and skin lesions, 35% each, followed by ligament injuries with 10% and fractures with 5%. Simple injuries were 83% in fitness skating, 70% in jumping/grinding and 60% in the halfpipe. Several visits were necessary for 4% of fitness skaters, 10% of jumpers/grinders and 23% in halfpipe injured persons. All four protectors were worn at the time of injury by 5% in fitness skating, 18% in jumping/grinding and 55% in the halfpipe. No protectors were worn in 26% of the fitness injuries, in 9% jumping/grinding and in 6% in the halfpipe. CONCLUSIONS: The results reveal an apparent discipline specific degree of danger and that fitness in-line skating is less dangerous than the resulting benefits for preventive medicine.},
